A question we get often: why can't we set up appointment confirmations and reminders to send from Aesthetix CRM the same way we handle marketing texts?
Here's the real answer.
The core issue: one SMS channel, no way to separate marketing from transactional
Aesthetix CRM sends all text messages through a single messaging channel. There is no built in way to separate a marketing text from a transactional one like an appointment reminder.
This matters because of how opt outs work. If a patient replies STOP to any text from your number, whether it was a promotional offer or a monthly newsletter blast, that STOP applies to everything going forward. That same patient will now also stop receiving their appointment reminders and confirmations, even though that was never their intent.
Put simply, a patient trying to unsubscribe from marketing messages ends up unintentionally unsubscribing from the messages that remind them to show up for their treatments.
Why this is a real risk
Appointment reminders exist to reduce missed appointments and keep your schedule full. If those reminders can be silently switched off by an unrelated marketing opt out, you lose visibility into who is actually getting reminded. A patient could miss a booked appointment and neither you nor they would know the reminder never went out.
This is not a small edge case. Practices run ongoing marketing campaigns and promotions through Aesthetix CRM, so the odds of a patient opting out at some point are real. Once that happens, their appointment communications go quiet with no clear warning.
What we recommend instead
Aesthetix CRM is built first as a marketing and sales platform, with patient engagement as a layer on top of that. It's excellent for nurture campaigns, promotions, review requests, and follow up communication where a shared opt out list is expected and appropriate.
Appointment confirmations and reminders should come from a system built to treat them as transactional, meaning they are not affected by marketing consent status. This is typically your scheduling software or EMR, since those systems are designed to separate patient care communications from marketing communications entirely.
Keeping these two functions in separate systems means a marketing opt out never puts a patient's appointment reminders at risk, and your practice keeps a clean, reliable way to confirm every booked visit.

We don't recommend sending appointment confirmations out of Aesthetix CRM because there is no way to keep marketing opt outs from also blocking those reminders. Keeping appointment confirmations in your scheduling or EMR system protects that communication, while Aesthetix CRM stays focused on what it does best: marketing, sales, and patient engagement around it.
