The Boulevard integration is available two ways. AX Connect is the full integration. Zapier is still available, but it runs the basic appointment and order workflows only and doesn't include the features we've added to the AX Connect integration.
Feature | AX Connect (Recommended) | Zapier |
|---|---|---|
HIPAA compliant | Yes | No |
Requirement | Boulevard Enterprise plan with API access | Paid Zapier account |
Pricing | $99/month integration fee to Aesthetix CRM | $500 one-time setup fee + paid Zapier plan |
Appointment created, completed, rescheduled and order completed | Included | Included |
Cancellations | Included, with no-show detection and opportunity moves | Basic contact update only |
Merged and mistaken bookings ignored | Included | Not available |
Dates in your location's time zone | Included | Not available |
Duplicate protection (orders counted once) | Included | Not available |
EMR Appointments calendar | Included | Not available |
Create client in Boulevard | Included | Not available |
Custom webhook triggers | Available (custom build pricing) | Not available |
Recommendation: We always recommend AX Connect with Boulevard's Enterprise plan. It is HIPAA compliant and includes everything described in this article. Zapier is not HIPAA compliant and does not include the cancellation, duplicate-protection or calendar features.
If you go the Zapier route, you'll need a paid Zapier plan with enough tasks for your volume. Multiply your average monthly appointments by 8.
Example: A practice with 200 appointments per month needs a Zapier plan with at least 1,600 tasks.
For how Zapier connects to Aesthetix CRM, see Zapier Integration.
Boulevard sends Aesthetix CRM a webhook every time an appointment or order changes. AX Connect receives it, checks it, pulls extra details from Boulevard where needed, and updates the contact in Aesthetix CRM.
Before anything is written, every event is checked:
Only real changes are sent. Boulevard sometimes sends the same event twice or sends an update where nothing changed. Those are skipped, so a contact isn't updated, noted or moved twice for the same thing.
A finished visit stays finished. If a booking webhook arrives after the appointment was already completed, cancelled or marked a no-show, it is ignored and the contact is not pulled back to Booked.
Orders are counted once. An order is added to Patient Lifetime Value one time, even if Boulevard delivers it twice.
Dates use your location's time zone. An evening appointment is dated the day it happens, wherever your location is.
Staff blocked time is ignored. Boulevard Timeblocks are not patient appointments and never reach the CRM.
Contact details are checked. Placeholder emails and phone numbers (for example [email protected] or 555-555-5555) are not sent, so different patients are never merged into one contact. A patient with neither a valid email nor a valid phone number is skipped.
Most updates reach Aesthetix CRM within seconds. New bookings and reschedules wait about one minute first, so that a cancellation or visit entered at the same moment is applied in the right order.
What triggers it: A new appointment is created in Boulevard (by staff or through online booking).
What happens:
The appointment date is formatted in your location's time zone.
Every service on the appointment is listed as one comma-separated value (for example "Botox, Filler, Hydrafacial").
The contact is created or updated in Aesthetix CRM and added to the Appointment Booked workflow.
The appointment is added to the EMR Appointments calendar as booked (AX Connect).
Fields updated in Aesthetix CRM:
First Name, Last Name, Email, Phone
Next Visit Date
Next Visit Service
Lead Status (Booked)
BLVD Client ID
BLVD DOB
BLVD Location Name
BLVD Service Name
Workflow actions in Aesthetix CRM:
A contact note is added with the service, status and appointment date.
An Appointment Booked tag is added.
The contact's opportunity is updated to the Booked stage in the Lead 2 Patient pipeline.
What triggers it: An appointment is marked as completed in Boulevard.
What happens:
The appointment services are formatted.
The contact is created or updated in Aesthetix CRM and added to the Appointment Completed workflow.
The calendar event is updated to showed (AX Connect).
Note: If Boulevard marks the appointment as a no-show or late cancellation, it goes down the cancellation path below instead, even when Boulevard sends it as "completed".
Fields updated in Aesthetix CRM:
First Name, Last Name, Email, Phone
Last Visit Date
Last Visit Service
BLVD Client ID
BLVD DOB
BLVD Location Name
BLVD Service Name
Workflow actions in Aesthetix CRM:
A contact note is added with the status and service.
The Appointment Booked tag is removed.
If the contact was a lead, their Contact Type is changed to Patient and a Patient tag is added.
The opportunity is updated to the Patient stage with a won status.
The Patient Lifetime Value is applied to the opportunity value.
What triggers it: An order is completed in Boulevard. This includes every point-of-sale transaction: services, memberships, gift cards, retail products and anything else processed through Boulevard's checkout.
What happens:
We request the full order from Boulevard by its order ID, then the client's details.
The order total is converted to dollars.
The contact is created or updated in Aesthetix CRM and added to the Order Paid workflow.
Each order is sent once. A repeat delivery of the same order is skipped, so Patient Lifetime Value is never counted twice.
Fields updated in Aesthetix CRM:
First Name, Last Name, Email, Phone
Invoice Paid Amount
BLVD Client ID
BLVD Location Name
Workflow actions in Aesthetix CRM:
A contact note is added with the order line items and invoice paid amount.
The contact is added to the Boulevard Birthday workflow (see Birthday Field Sync below).
The Invoice Paid Amount is added to Patient Lifetime Value.
The existing opportunity value is updated with the Patient Lifetime Value.
Orders don't appear on the EMR Appointments calendar; only appointments do.
What triggers it: An appointment is cancelled in Boulevard, or marked as a no-show.
What happens (standard setup, AX Connect):
Boulevard's own cancellation reason tells us whether it was a no-show or a cancellation (client, late or staff cancellation).
Merged and Mistake cancellations are ignored. These are housekeeping (two bookings combined, or an appointment booked in error), not a patient cancelling, so the contact and opportunity are left alone.
The contact is updated and Lead Status is set to Canceled or No Show.
A contact note records whether it was a cancellation or a no-show, with the service and date.
The contact's most recently updated opportunity in the Lead 2 Patient pipeline is moved to Cancel / No Show, but only while it is still at an early stage: Lead, New Lead, Replied, Pre-Qualify Call or Booked. An opportunity at Consult Complete, Patient or a later stage is never moved backwards.
The calendar event is updated to cancelled or no-show. A Merged or Mistake appointment is marked invalid so it doesn't count in your reports.
Your practice may vary. Some practices have a custom cancellation setup, for example one that adds a cancellation tag for a rebooking campaign, or that uses separate surgical and non-surgical pipelines. If you're not sure how cancellations work in your account, contact support.
Cancellation follow-up: The standard setup does not add a tag, so it doesn't start a rebooking campaign on its own. If you'd like cancelled or no-show patients to enter a follow-up workflow, let us know and we'll set it up.
What triggers it: An appointment is moved to a new date, time or service in Boulevard.
What happens:
A reschedule is handled like a new booking: the contact goes back through the Appointment Booked workflow with the new date and service.
It's sent only when something you'd see in the CRM changed (date, time, service, staff or location). Boulevard also sends reschedule events for small edits; those are skipped.
The same calendar event moves to the new time; no second event is created.
Every Boulevard appointment is copied onto a calendar in your Aesthetix CRM account called EMR Appointments. It lets the CRM's calendar view and appointment reports show what actually happens in Boulevard: how many appointments were booked, completed, cancelled and missed.
How it's set up:
Practices with one Boulevard location get one calendar, EMR Appointments.
Practices with several locations get one calendar per location, named EMR Appointments ยท [location name] (for example, EMR Appointments ยท Downtown), each in its own color so locations are easy to tell apart on the calendar view.
What each event shows:
The services booked, followed by the staff member(s)
The appointment's start and end time
The patient's contact record (the appointment also appears in the contact's appointment history)
A status that follows Boulevard:
In Boulevard | On the EMR Appointments calendar |
|---|---|
Booked or rescheduled | Confirmed (rescheduled appointments move to the new time) |
Completed | Showed |
Cancelled | Cancelled |
No-show | No-show |
Merged or Mistake | Invalid (not counted) |
The calendar is silent. It never sends confirmations, reminders or cancellation messages to patients, and never alerts staff or assigns contacts. Your patients keep receiving only the messages Boulevard and your existing workflows send. We check every day that the calendar is still set up this way.
Important: Please don't change the calendar's notification settings, and don't book patients into the EMR Appointments calendar by hand. An appointment booked there is not sent to Boulevard, and changes made to synced events in the CRM are replaced the next time the appointment changes in Boulevard. Book in Boulevard as usual.
What's included: appointments created or changed in Boulevard from late October 2026 onward. Older appointments appear once they next change in Boulevard.
The calendar itself sends nothing, but your workflows can still react to it. Because Boulevard appointments are now real appointments on a CRM calendar, workflow triggers behave like this:
Workflow trigger | Fires for Boulevard appointments on the EMR Appointments calendar? |
|---|---|
Appointment Status with no calendar filter | Yes. It fires when an appointment is added (status Confirmed) and each time its status changes (Showed, Cancelled, No-show, Invalid). |
Appointment Status filtered to another calendar ("In Calendar") | No |
Appointment Status filtered to an EMR Appointments calendar | Yes, on purpose |
Customer Booked Appointment | No. This trigger is for bookings made through a booking link or widget. |
What this means for your account:
Check your own appointment workflows. If you built a workflow on the Appointment Status trigger without an "In Calendar" filter, it now also runs for every Boulevard appointment. That could mean a second reminder or a second follow-up for patients who already get one from Boulevard. Add an In Calendar filter for the calendars the workflow is meant for.
The standard Aesthetix CRM workflows are not affected. The appointment workflows we install are limited to their own calendars, and the Boulevard workflows run from the integration, not from calendar triggers.
You can use it on purpose. A workflow with the Appointment Status trigger filtered to your EMR Appointments calendar (for example, status No-show) is a way to build automations from Boulevard appointment statuses. Contact support if you'd like help setting one up.
What triggers it: A tag called blvd-create is added to a contact in Aesthetix CRM (manually or by automation).
What happens:
The tag triggers a workflow in AX Connect that sends a create client request to Boulevard.
The client is created in Boulevard.
Note: This feature is only available with AX Connect and is included in your standard integration fee.
Beyond the core workflows, the calendar and client creation, our Boulevard app supports additional webhook triggers that can be built for your practice. These require custom quoting and are not included in the standard integration fee. To request one, contact support and submit a Custom Build Request.
If you use Boulevard for your membership program, we can listen for:
Membership Created: start automations when a new membership is sold
Membership Cancelled: start follow-up or retention workflows
Membership Reactivated: start re-engagement automations
Membership Renewal Succeeded: track ongoing membership revenue
Client Created in Boulevard: create the contact in Aesthetix CRM automatically
Client Updated in Boulevard: update the contact in Aesthetix CRM automatically
Client Marketing Preference Updated: sync DND (Do Not Disturb) between Boulevard and Aesthetix CRM. If a client opts out in Boulevard, their DND settings in Aesthetix CRM are updated automatically.
Tag Added in Boulevard: start workflows in Aesthetix CRM from tags you add to clients in Boulevard
Tag Removed in Boulevard: start workflows when tags are removed
Referral Reward Created: know when a referring client earns a reward, so you can build automations around Boulevard's referral program
List Appointments: pull appointment data from Boulevard
Update Appointments: change appointments in Boulevard from Aesthetix CRM
Extra Boulevard data (for example the client's address) can be mapped into your CRM fields on request.
The integration can't write to the CRM's native Date of Birth field directly. Instead, it writes the birthday from Boulevard into a custom field called BLVD DOB.
A dedicated Birthday workflow then copies the value from BLVD DOB into the native Date of Birth field, so any birthday automations you've built in the CRM work with Boulevard data.
The Birthday workflow starts when a contact enters the Order Paid workflow.
Several workflows in your Aesthetix CRM account are hidden and locked on purpose. They contain the core integration logic, and changing them could break the integration:
Boulevard Appointment Booked
Boulevard Appointment Completed
Boulevard Order Paid
Boulevard Birthday
If you need custom automation built on top of any of these, let us know. We can unlock specific workflows or help you build triggers from the tags and fields these workflows create.
All Boulevard-specific fields are in a Boulevard folder within your custom fields.
Custom Field Name | Type | Custom Value | Updated on |
|---|---|---|---|
BLVD Client ID | Text |
| Every event |
BLVD Location Name | Text |
| Every event |
BLVD Service Name | Text |
| Appointment created, rescheduled, completed |
BLVD DOB | Date |
| Appointment created, rescheduled, completed |
BLVD Appointment Date | Date |
| Appointment created or rescheduled, for practices set up to fill it |
BLVD Last Appointment | Date |
| Appointment completed, for practices set up to fill it |
BLVD Appointment Count | Numerical |
| Not filled in (see note) |
Note: BLVD Appointment Count exists in your account, but the integration doesn't fill it in. BLVD Appointment Date and BLVD Last Appointment are filled for some practices, depending on how the integration was set up for your account. For the visit date most automations should use, see the Next Visit and Last Visit fields below.
The integration also updates these standard Aesthetix CRM fields (see Next Visit & Last Visit Fields for how the visit fields work):
Field | Updated on |
|---|---|
First Name, Last Name, Email, Phone | Every event (only valid emails and phone numbers) |
Next Visit Date, Next Visit Service | Appointment created or rescheduled |
Last Visit Date, Last Visit Service | Appointment completed |
Lead Status | Booked on appointment created or rescheduled; Canceled or No Show on cancellations and no-shows |
Invoice Paid Amount | Order completed |
Patient Lifetime Value | Order completed (through the Order Paid workflow) |
Contact Type | Appointment completed (lead becomes Patient) |
Address fields are not synced by default. City, State, Address and Postal Code are not filled in from Boulevard unless address mapping was set up for your practice. If you'd like them, see Practice-Specific Fields in Custom Build Options above.
Your account may have additional Boulevard fields that were mapped for your practice.
Zapier is not HIPAA compliant. We always recommend AX Connect with Boulevard's Enterprise plan. If you use Zapier, your data passes through a platform that is not HIPAA compliant, and the cancellation, duplicate-protection and calendar features are not available.
Multi-service appointments come in as one value. Several services appear as a comma-separated list (for example "Botox, Filler, Hydrafacial") in the service fields and in the calendar event title.
Patients without a valid email or phone are skipped. The CRM matches contacts by email and phone, so an appointment for a client with neither is not sent.
Bookings and reschedules arrive about a minute later than other updates. The short wait keeps updates in the right order when a cancellation or visit is entered at the same time.
A rare delivery error can miss one update. If AX Connect can't process a Boulevard event (this is uncommon), that one update is missed. The appointment's next change in Boulevard brings the contact and calendar up to date.
The calendar starts from late October 2026. Appointments that haven't changed since then don't appear on the EMR Appointments calendar until they next change in Boulevard.
Opportunities are moved forward, never backwards. A cancellation only moves an opportunity that is still at Lead, New Lead, Replied, Pre-Qualify Call or Booked.
Create client in Boulevard is AX Connect only. The blvd-create tag is not available through Zapier.
Custom triggers require additional quoting. Membership webhooks, client sync, marketing preference sync, tag triggers and referral rewards are available as custom builds.
Birthday uses a workaround. The integration writes BLVD DOB, and the Birthday workflow copies it into the native Date of Birth field.
How do I know the integration received an appointment?
Open the contact's Notes tab. Bookings, completed visits, cancellations, no-shows and orders each add a note. With AX Connect, you can also find the appointment on the EMR Appointments calendar. Repeated or unchanged Boulevard events are skipped on purpose, so they don't add another note.
Which integration option should I choose, AX Connect or Zapier?
AX Connect. It's HIPAA compliant and includes no-show detection, duplicate protection, the EMR Appointments calendar, client creation in Boulevard and custom triggers. Zapier runs the basic workflows only and is not HIPAA compliant.
How do I calculate how many Zapier tasks I need?
Multiply your average monthly appointments by 8. For example, 200 appointments a month ร 8 = 1,600 tasks.
What happens when a patient cancels or doesn't show up?
Lead Status changes to Canceled or No Show, a note is added, and if the opportunity is still at an early stage (Lead, New Lead, Replied, Pre-Qualify Call or Booked) it moves to Cancel / No Show. The calendar event shows cancelled or no-show. Some practices have a custom cancellation setup; contact support if you're not sure how yours works.
Why didn't a cancellation move the opportunity?
Either the opportunity was already past booking (for example at Consult Complete or Patient), it was already at Cancel / No Show, or the cancellation was a Merged or Mistake cancellation in Boulevard, which is ignored.
Will the EMR Appointments calendar send reminders to my patients?
No. It's silent: it never sends confirmations, reminders or cancellation messages, and never alerts staff. Please don't turn its notifications on.
Will my appointment workflows run for Boulevard appointments?
Workflows using the Appointment Status trigger without an "In Calendar" filter will, because Boulevard appointments are now on a CRM calendar. Add an In Calendar filter to any workflow that should only run for your other calendars. Workflows using Customer Booked Appointment are not affected.
Can I book appointments on the EMR Appointments calendar?
No. Book in Boulevard as usual. An appointment booked directly on the EMR Appointments calendar isn't sent to Boulevard, and synced events are updated from Boulevard automatically.
Why don't I see older appointments on the EMR Appointments calendar?
The calendar includes appointments created or changed in Boulevard from late October 2026 onward. Older appointments appear when they next change in Boulevard.
We have several locations. How do I tell their appointments apart?
Each Boulevard location has its own EMR Appointments calendar with its own name and color. The BLVD Location Name field on the contact also shows the location of their latest appointment.
What happens when an appointment is rescheduled?
It's treated like a new booking: the contact goes back through the Appointment Booked workflow with the new date and service, and the calendar event moves to the new time.
Can I track memberships from Boulevard?
Yes. Membership purchases processed through Boulevard's checkout are captured by the Order Completed workflow and added to Patient Lifetime Value. For detailed membership tracking (created, cancelled, reactivated, renewal succeeded), we offer custom membership webhook builds on AX Connect.
How do I create a client in Boulevard from Aesthetix CRM?
Add the blvd-create tag to the contact. This is available on AX Connect only and is included in your standard integration fee.
Can I sync DND / marketing preferences between Boulevard and the CRM?
Yes, through the Client Marketing Preference Updated webhook, available as a custom build on AX Connect. Contact support for a quote.
If something in Boulevard isn't showing up in Aesthetix CRM the way you expect, contact Aesthetix CRM support through the help widget and tell us the appointment's date and location so we can trace it. For membership, tag, marketing-preference or other custom triggers, submit a Custom Build Request and we'll quote it for your practice.
