When you need form and survey responses in a spreadsheet (for example, to build reports, share data with your team, or keep an offline record), you can export your submissions and work with them in a tool like Google Sheets.
Exported submissions include the answers your patients provided so you can sort, filter, and analyze intake and survey data outside the platform.
Note: Export the data you need on a regular cadence if you rely on spreadsheets for reporting, since a spreadsheet is a point-in-time snapshot and will not update automatically as new submissions come in.

Form submissions can be viewed from any area where you open the appointment window. Whether you're working from the Calendars tab, Contacts tab, or Inbox, you will have consistent access to the form data linked to appointments.
To view form submissions:
Navigate to the Calendars, Contacts, or Inbox area.
Click on an appointment to open the appointment window.
If the appointment includes a custom form, a Forms tab will appear in the window.
Select the Forms tab to view the full form submission in a structured layout that mirrors the Forms module.
This feature supports custom forms that are added to the calendar as part of a booking process. These are the same forms you create and manage through the Forms Builder.

Why don't I see the Forms tab in my appointment window?
The Forms tab only appears if the appointment includes a custom form submission. Appointments created with the default booking form will not display the tab.
Can I edit the form submission from the appointment window?
No, the form submission is displayed as read-only. To make changes, go to the Forms module or edit the form within the contact record.
Where does the form data come from?
It comes from the form attached to the calendar booking or added during the appointment workflow.
Is support for default booking form submissions coming?
Yes, the ability to view default booking form submissions in the Forms tab is in development and will be added in an upcoming release.
Can I access the Forms tab from the Inbox or Contacts tab?
