Creating a Patient in Elation EMR from Aesthetix CRM

This guide walks through how to properly create a patient record in Elation EMR via Aesthetix CRM.

Required Fields

Before creating the patient, ensure the following fields are completed in the contact profile:

• First Name

• Last Name

• Email

• Phone Number

• Date of Birth

• Gender

• Street Address

• City

• State

• Postal Code

• Primary Physician (custom field)

✅ Note on Primary Physician:
The name selected must match exactly how it appears in Elation EMR, since the data is sent through the API.

Steps to Create the Patient

1. Open the contact profile in Aesthetix CRM.

2. Ensure all required fields above are filled out and saved.

3. Scroll down to the Tags section.

4. Type and apply the tag: Elation-Create.

Within 30–45 seconds, the patient will be created in Elation EMR.
