# How to Create a Patient in Dr. Chrono From Aesthetix CRM

Learn how to add patients to Dr. Chrono from Aesthetix CRM

This guide outlines the steps for creating a patient record in **DrChrono** directly from **Aesthetix CRM**.

### Required Fields

Before pushing the contact to DrChrono, ensure the following fields are completed in the contact’s profile:

-   **First Name**
    
-   **Last Name**
    
-   **Email**
    
-   **Phone**
    
-   **Gender**
    
-   **Date of Birth**
    

> ✅ _If your practice has multiple providers:_  
> There will also be a **custom field** such as **Doctor Name** or **Provider Name**.

-   This field must match _exactly_ how the provider’s name appears in DrChrono so that the patient is correctly associated with the right provider.
    

### Creating the Patient

Once all required information is entered:

1.  **Open the contact profile** in Aesthetix CRM.
    
2.  **Scroll down to the Tags section.**
    
3.  **Apply the tag:**  
    Type and select `DC-Create`.
    

Within **a few minutes**, the patient will be created in DrChrono from Aesthetix CRM.

-   Always double-check that all required fields are filled out and saved before applying the `DC-Create` tag.
    
-   Consistency in provider names ensures the patient is correctly assigned in DrChrono.
